The RGPs of Ontario is leading the development of measures, indicators and related data collection tools specific to older people living with frailty and their caregivers. The focus of this work is the development of a set of common, core outcome metrics aligned with the Quadruple Aim Framework and the work of Specialized Geriatric Services (SGS) across Ontario.
The objectives of our activity in this area include:
- Supporting SGS programs and services in Ontario in reporting, quality improvement, evaluation, and decision-making by developing common, evidence-informed core metrics that align with provincial and national priorities;
- Determining data sources and infrastructure required to collect and report data;
- Developing a feedback reporting system that informs quality improvement, evaluation and decision making in SGS programs and services locally, and the RGPs of Ontario collectively.
